Billing

We automatically bill your card each month recurring on the day of your sign up. You can book an appointment anytime and your discount will be automatically applied when you check out after your service. Membership does not include gratuity. If your card on file is declined, we will reach out to ask for a new card. If we do not receive a response within 30 days, your membership will be canceled and any remaining facials will be forfeited.

Minimum Commitment

Membership begins with a 4-month commitment to experience the benefits of regular services. After that, membership is month-to-month and can be ended at any time without penalty or charge. We do not accept cancellations over the phone or verbally. In order to cancel, you must email at least 15 days before your next billing cycle. Not sure which email? Please refer below. Once your membership is canceled, any remaining services will be forfeited.

Usage Policy

Membership services must be used within the month in which they are paid. Unused services will not accumulate or transfer to the following month.

Gifting

Gift your month's benefit to a friend or family member twice in a calendar year.

Freezing

We know life can get crazy, which is why you are able to freeze your account for up to 3 months a calendar year. All Freeze requests must be done in writing, emailed to freeze. Not sure which email? Please refer below.

Membership Discounts

Membership discounts do not apply retroactively and are only valid the day you start your membership. Discounts cannot be shared and cannot be combined with other offers unless specified otherwise. Member discounts are not valid on product purchases on our online shop.

This consultation is to design a personalized surgical plan together with Dr. Jose Aquino.

The cost of the virtual consultation is $200, and you have the right to come for an in-person consultation with Dr. Jose Aquino within 6 months after the virtual consultation.

The cost of the consultation is to properly compensate for the time, medical expertise, and recommendations of Dr. Aquino. Once the questionnaire with the requested information is submitted, you will be contacted within 2 business days to schedule the appointment.

With the intention to prepare a personalized diagnosis and receive you with a plan and approach designed for you, we request to fill a questionnaire and attach some photos according to the procedure to be performed as part of the evaluation.

* If you are not requesting facial surgery, please do not show your face. These images are 100% confidential and for private and medical use only.

Payment:

All patients are required to pay the full consultation fee at the time of scheduling the appointment. This fee is non-refundable.

Non-Refundable:

Please note that the cost of the medical consultation is non-refundable once the service has been provided. This includes any charges related to assessment, diagnosis, treatment, or any other medical service provided during the consultation.

Cancellations:

If a patient needs to cancel or reschedule their appointment, they must provide at least 24 hours' notice. Failure to do so will result in the forfeiture of the consultation fee.

No-shows:

If a patient fails to show up for their appointment without providing any notice, the consultation fee will be non-refundable.

Exceptions:

In case of emergencies or unforeseen circumstances, the healthcare provider may consider individual cases for exceptions. Such exceptions will be solely at the discretion of the healthcare provider.
